Up until now the only way to participate in the Cosmic Citizenship Quiz was by using the online version listed among "Key Resources" on the Urantia Book Fellowship website here: http://www.urantiabook.org/index-urantia-book-links.htm
However, the problem with the online version (and the subject of most of the feedback I received) was that it relies on the Google infrastructure, i.e. it forces one to have (and login to) the Google account and allow everyone to see their Google email address. This is obviously not very "privacy-friendly", especially in such a sensitive area as one's ignorance in the area which one professes to have been "a reader for many decades".
Another obvious disadvantage of the online version is that, you have to be, well... online in order to use it 
So, I have implemented a totally-private and offline version of the same functionality by embedding the Cosmic Citizenship Quiz inside the PDF files of the British Study Edition of the Urantia Papers. So, from now on, all PDF flavours (except KindleDX two-column one) contain the quiz:

To use it you do NOT need any special software. It works everywhere, i.e. computers, tablets, mobiles and, last but not least, eInk devices like Kobo and Kindle (and ALL others). I mention Kobo and Kindle especially because on these two families of devices you can enjoy the best PDF/DjVu (and ALL other formats) viewing software on this planet called KOReader (see https://github.com/koreader/koreader) which is especially optimized for reading the Urantia Book on such devices. But really, all you need to use the Quiz is just the ability to turn the pages, i.e. nothing (as, obviously, every software that "reads PDF" files allows you to turn the pages in one way or another).
For ideal enjoyment of the quiz you should configure your PDF reader to disable continuous scrolling. In Adobe Reader this means selecting "View/Page Display/Single Page" menu option. On Android I recommend "ezPDF Reader" which also allows to disable continuous scroll. This gives an "animation effect" impression because the ticks and crosses appear against the correct and wrong answers respectively.
To jump to the quiz you simply click (or touch if appropriate) on the title of the Paper (or Foreword) in the text. This would jump to that Paper's section in the Quiz. After finishing all the questions for that Paper you are taken to the next Paper's section in the quiz, clicking (or touching) which you are then taken back to that (i.e. next) Paper's text section. So, this assumes the following workflow:
1. read a Paper 2. pass a quiz on that Paper 3. read the next Paper 4. pass a quiz on that Paper 5. and so on...
Also, if your PDF viewer software supports following links (all three of Adobe Reader, ezPDF Reader and KOReader do) then you might enjoy clicking on your choice answer (when answering a question) which leads to the next page (i.e. the page with answers) instead of simply turning the page by some other means.

At the moment there are 808 questions in total (the online version has only 743), but I haven't sorted all of them into the relevant Papers yet. So, slightly less than 50% of the questions are still appended "unsorted" at the end of the book, i.e. they seem to belong to Paper 196 but really they should be spread across all of them.
